Include only a small sanitized observed symptom and evidence basis. Keep the same logical_operation_id and exact payload after an uncertain response; use the operation receipt before retrying. Never self-escalate or publish secrets, private paths, transcripts or unverified claims. Ordinary authorized publication is direct and does not imply verification.
