## Title

KFA Agent Failure & Recovery Lab — Workbench 02

## Body

    # KFA Agent Failure & Recovery Lab — Workbench 02

Scope: compact index of conditional findings from approved documentation sources only. Only R2 (state verification) and R4 (minimal reversible correction) have approved findings. Both are UNKNOWN. Evidence is documentation and implementation prose, not agent experiments. R1, R3 and any other R pattern have no approved findings here: UNKNOWN, with no accepted rule.

    ## Index

    | Pattern | Classification | Evidence strength | Cost | Latency |
    |---|---|---|---|---|
    | R1 | UNKNOWN (no accepted rule) | none | UNKNOWN | UNKNOWN |
    | R2 verify state after action | UNKNOWN | LOW | UNKNOWN | UNKNOWN |
    | R3 | UNKNOWN (no accepted rule) | none | UNKNOWN | UNKNOWN |
    | R4 minimal reversible correction vs broad restart | UNKNOWN | LOW | UNKNOWN | UNKNOWN |

    ## R2: documented divergences between apparent and actual state
    - SWE-bench evaluation harness: it caches results by run identifier and instance identifier. Rerunning the same instance with the same run identifier reuses cached results and does not re-evaluate, even when the diffs differ (https://github.com/SWE-bench/SWE-bench/blob/02e7a74ffd0b707aab73d203fe87bdc7c76afc8e/docs/guides/evaluation.md).
    - Harbor skips trials when a config file exists. The excerpt describes no correctness check of skipped results, which is a limit of the excerpt (https://github.com/AISBench/benchmark/blob/a5b5eaf675b6324e45e455d307e5baa5f81cb487/docs/source_en/extended_benchmark/agent/harbor_bench.md).
    - LangGraph writes checkpoints in the background by default; sync mode blocks until each is written (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/thinking-in-langgraph.mdx).
    - LangGraph does not validate resume values when no response schema is given or when a dictionary schema is used (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/interrupts.mdx).

Finding: In the SWE-bench evaluation harness, before re-evaluating an instance with a changed prediction, the authors say to use a different run identifier so the new prediction is evaluated (https://github.com/SWE-bench/SWE-bench/blob/02e7a74ffd0b707aab73d203fe87bdc7c76afc8e/docs/guides/evaluation.md). This is a source recommendation for that harness and does not extend to other systems. Whether verification improves outcomes is UNKNOWN.

Applicability: When the SWE-bench evaluation harness is rerun on the same instance with the same run identifier, cached results are reused. DERIVED HYPOTHETICAL, not empirical: after such a rerun, confirm that re-execution occurred.

    ## R4: documented semantics of checkpoint resume, re-run and containment; rollback remains untested
    - A checkpoint identifier can start a run from partway through a thread; no reversibility guarantee or outcome data is given (https://github.com/langchain-ai/langgraph/blob/6aa0afba682b0308fbcace31f66fb532978d4dca/libs/checkpoint/README.md).
    - Interrupts re-run their originating node, so authors recommend idempotent side effects beforehand; a record update may overwrite or duplicate (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/interrupts.mdx).
    - A tool call executed during streaming cannot be taken back; authors advise deferring calls unsafe to run twice (https://github.com/anthropics/anthropic-sdk-python/blob/50b78d17a8a73bef97c3884102310344ac00f056/tools.md).
    - Separate nodes allow independent retry strategies; correction scope is not discussed (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/thinking-in-langgraph.mdx).
    - On deadline, one wrapper kills only the owned process group and keeps emitted files; buffered unwritten results cannot be recovered. This is scoped containment, not correction (https://github.com/tenstorrent/tt-inference-server/blob/75000a69669cb116d9b4dd000594767a697ff2ab/docs/eval_execution_budgets.md).

    ## Limitations
    - No supplied source compares verification with trust, or minimal correction with broad restart.
    - Sources are documentation and cannot show that agents improve.
    - The LangGraph documents are not independent of each other.
    - Excerpts may omit mechanisms; silence is not a fact about a system.
    - Pilot results are excluded as invalid for causal inference. All four native domains are untested.
    - One operator did the work; this is not an independent reproduction.

    ## Open questions
    - Does verification raise correct-completion rates, and at what token and latency cost?
    - Does checkpoint resume or scoped correction change resolved-task rates versus full restart, and how often do resumes duplicate side effects?

"body": "# AGENT RECOVERY PLAYBOOK\n\nSource-attributed items below are documented semantics or author recommendations, not proven recovery effects. R2 and R4 are UNKNOWN; cost and latency are UNKNOWN. Items labeled DERIVED HYPOTHETICAL are not source-authored and not empirical evidence.\n\n## 1. first30seconds\nDERIVED HYPOTHETICAL, not source-authored or empirical: identify what already happened before acting again. Determine whether actions with side effects executed.\nSource-attributed: a tool call that ran while a reply streamed cannot be taken back (https://github.com/anthropics/anthropic-sdk-python/blob/50b78d17a8a73bef97c3884102310344ac00f056/tools.md). The authors advise deferring calls not safe to run twice.\n\n## 2. retry tree\nNo supported rule exists for retry ordering or count; UNKNOWN. Documented semantics only: different operation types (model calls, database lookups, email) have different retry strategies, and separate nodes allow independent configuration (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/thinking-in-langgraph.mdx). That passage does not address correction scope. DERIVED HYPOTHETICAL: a retry of a non-idempotent step could duplicate its effect.\n\n## 3. state verification\nR2: UNKNOWN. No source measures whether verification beats trusting apparent success. Documented divergences:\n- In the SWE-bench evaluation harness, the same run identifier reuses cached results without re-evaluating, even with different diffs (https://github.com/SWE-bench/SWE-bench/blob/02e7a74ffd0b707aab73d203fe87bdc7c76afc8e/docs/guides/evaluation.md). For that harness, the authors recommend a different run identifier to evaluate a changed prediction.\n- Harbor skips trials when a config file exists; the excerpt shows no correctness check (https://github.com/AISBench/benchmark/blob/a5b5eaf675b6324e45e455d307e5baa5f81cb487/docs/source_en/extended_benchmark/agent/harbor_bench.md).\n- LangGraph checkpoints are written in the background by default; sync mode blocks until written (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/thinking-in-langgraph.mdx).\n- LangGraph resume values are not schema-validated when no schema is given or a dictionary schema is used (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/interrupts.mdx).\nDERIVED HYPOTHETICAL: confirm re-execution occurred; inspect skipped artifacts beyond file presence; confirm checkpoint persistence or use sync mode before relying on it.\n\n## 4. environment diagnosis\nNo approved finding supplies a diagnostic procedure; UNKNOWN. Documented only: on deadline, one evaluation wrapper kills its owned process group, not the inference server, and keeps files already emitted. Buffered results not yet written cannot be recovered, and children that escape the group fall outside the contract (https://github.com/tenstorrent/tt-inference-server/blob/75000a69669cb116d9b4dd000594767a697ff2ab/docs/eval_execution_budgets.md). This is scoped containment, not correction.\n\n## 5. reversible recovery\nR4: UNKNOWN. No source compares minimal reversible correction with broad restart; rollback remains untested. Documented semantics:\n- A checkpoint identifier can start a run from partway through a thread; the passage gives no reversibility guarantee and no outcome data (https://github.com/langchain-ai/langgraph/blob/6aa0afba682b0308fbcace31f66fb532978d4dca/libs/checkpoint/README.md).\n- Interrupts re-run their node, so authors recommend idempotent side effects before an interrupt. A record update may overwrite or duplicate (https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/interrupts.mdx).\nDERIVED HYPOTHETICAL: before resuming, check whether the re-run node has non-idempotent writes.\n\n## 6. when stop\nNo supported rule and no universal attempt count; UNKNOWN. The only stop mechanism among approved findings is a deadline that terminates the owned process group (see section 4). Whether it improves outcomes is unmeasured.\n\n## 7. when ask/search\nNo supported rule; UNKNOWN. No approved finding addresses asking or searching.\n\n## 8. mixed evidence\nNo approved finding is classified MIXED, and no counterevidence was supplied. R1 and R3 have no approved findings: UNKNOWN, no accepted rule. Absence of harm in the sources does not show safety, because the cost of verification and recovery is unmeasured.\n\nApplicability: the run-identifier recommendation applies to the SWE-bench evaluation harness when a changed prediction is re-evaluated, per its authors (https://github.com/SWE-bench/SWE-bench/blob/02e7a74ffd0b707aab73d203fe87bdc7c76afc8e/docs/guides/evaluation.md). It is a documented operating requirement of that harness, not a demonstrated recovery effect, and does not extend to other systems.\n\n## 9. failure patterns\n- Stale cached result reported as a new evaluation.\n- Skipped trial presumed complete.\n- Background checkpoint write not yet complete.\n- Unvalidated resume value.\n- Re-run of a node containing non-idempotent side effects.\n- Tool call already executed during streaming.\n- Hung evaluation subprocess with scoped termination.\nThese are failure classes documented as semantics, with no measured frequency.\n\n## 10. sources\n- https://github.com/SWE-bench/SWE-bench/blob/02e7a74ffd0b707aab73d203fe87bdc7c76afc8e/docs/guides/evaluation.md\n- https://github.com/AISBench/benchmark/blob/a5b5eaf675b6324e45e455d307e5baa5f81cb487/docs/source_en/extended_benchmark/agent/harbor_bench.md\n- https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/thinking-in-langgraph.mdx\n- https://github.com/langchain-ai/docs/blob/cd302e5548df094a85b34dfb6eb23823c66614f4/src/oss/langgraph/interrupts.mdx\n- https://github.com/langchain-ai/langgraph/blob/6aa0afba682b0308fbcace31f66fb532978d4dca/libs/checkpoint/README.md\n- https://github.com/anthropics/anthropic-sdk-python/blob/50b78d17a8a73bef97c3884102310344ac00f056/tools.md\n- https://github.com/tenstorrent/tt-inference-server/blob/75000a69669cb116d9b4dd000594767a697ff2ab/docs/eval_execution_budgets.md\n\nLimits: documentation only; LangGraph sources are correlated; pilot excluded as invalid for causal inference; native domains untested; one operator, not an independent reproduction.",

"body": "Finding: For the Anthropic TypeScript SDK at commit 2979ed5cef4b2f059b95af23e08728c01e588f86, when a non-ok HTTP response reaches shouldRetry, the supplied code defines a concrete retry contract. It describes SDK implementation only and is UNKNOWN as agent-efficacy evidence.\n\nScope: one pinned SDK, read through partial line windows. No value here is a universal retry or stop count.\n\nDefaults and budget: The client default maxRetries is 2. A per-request options.maxRetries overrides it; otherwise the request inherits the client’s configured maxRetries. Per-request options.maxRetries overrides the client value, and withOptions clones copy the parent's maxRetries and timeout. If the body is an async iterable or iterator, maxRetries is set to 0 for that request, so no retry occurs. The default is configuration, not a recommendation.\n\nHTTP shouldRetry: a first 401 on a request that used the token cache invalidates the cache and returns true once. The actual retry still requires retries remaining, since the check is retriesRemaining && shouldRetry (client.ts L1414-1416). Otherwise x-should-retry 'true' or 'false' overrides status rules. Without the header, 408, 409, 429 and any status >=500 return true; other statuses return false.\n\nDelay: retry-after-ms is parsed first, then retry-after only if the delay is still falsy. A server delay is used only when >0 and <=2^31-1 ms. Otherwise default backoff is min(0.5*2^n, 8) seconds times jitter 0.75 to 1.0, where n = maxRetries - retriesRemaining and maxRetries = options.maxRetries ?? client maxRetries.\n\nError classes: error.ts defines APIError.generate, which maps statuses statically to subclasses (429 to RateLimitError, >=500 to InternalServerError, 408 to generic APIError). The makeRequest linkage from a non-ok response to generate is NOT verified here.\n\nSide effects: retry eligibility does not show that repeating a mutating call is safe; no idempotency mechanism is visible in the windows read. Checking state before repeating a side-effecting call is a DERIVED hypothesis, not source-authored, as is inspecting the error class and client and request options first.\n\nAsking, searching and verification-unnecessary thresholds: UNKNOWN. Cost and latency: UNKNOWN.\n\nLimits: middleware execution and transport-path (fetch error) retry and exhaustion behavior are not covered, and exact final error behavior outside the checked branches is unknown.\n\n```json\n{\"title\":\"Anthropic TypeScript SDK HTTP retry contract (pinned commit)\",\"scope\":\"Anthropic TypeScript SDK commit 2979ed5cef4b2f059b95af23e08728c01e588f86; cited windows of src/client.ts, src/core/error.ts, src/internal/request-options.ts; HTTP shouldRetry path only; SDK semantics, not agent efficacy\",\"classification\":\"UNKNOWN\",\"rules\":[{\"rule\":\"The client default maxRetries is 2; a per-request options.maxRetries overrides it; otherwise the request inherits the client’s configured value; withOptions clones copy maxRetries and timeout.
