A rejected async callback is reported to bypass onError and object.error and to surface as an unhandled rejection, and submit() can resolve while the callback is still pending (https://github.com/vercel/ai/issues/22576). Reported conditions: the reporter says a synthetic-fetch Vitest test needs no backend or credentials. It reportedly fails on the final assertions (onError called with the error, object.error equal to it) on current main. The reporter also says the issue appears with published Svelte modules compiled by the Svelte compiler, and in the repo's Svelte Vitest/jsdom config, on Node.js v26.7.0 and pnpm 11.23.0 (issue body). A synchronous throw from the same callback reportedly reaches the existing catch block, so the expected behavior is for async failures to follow that path. Cause (reported, not maintainer-confirmed): the reporter attributes it to the WritableStream close handler invoking this.#options.onFinish(...) without awaiting or returning it, so the enclosing pipeTo and submit catch never observe the rejection (issue body). A contributor comment says its reproduction saw submit() settle before a deferred onFinish finished and a rejected async callback not populate object.error (https://github.com/vercel/ai/issues/22576#issuecomment-6101745264). Fix status: the reporter proposed awaiting onFinish in close. A later contributor comment says a bugfix PR #22622 was opened that awaits the callback and adds regression tests for deferred and rejected async onFinish plus a patch changeset (https://github.com/vercel/ai/issues/22576#issuecomment-6102019053). The PR was not supplied, and no merge or release is evidenced, so this is a proposed or in-review fix, not a shipped one. Practical implication: if your Svelte app does async work such as saving in onFinish, failures may go unreported through onError/object.error on the affected version, and code awaiting submit() may continue before that work completes. Treat this as conditional on the reported version. Limitations: this is an external report with contributor comments; it was not reproduced by KFA.
