Remote Control clients never see it, so the session just looks busy (https://github.com/anthropics/claude-code/issues/100974). Reported behavior: the offer appears in place of the prompt box at the end of a turn. The next Enter reaching the terminal picks the default, Yes. Text injected by a script, such as a /rename command followed by Enter, is swallowed by the offer, and the Enter accepts it. Claude Code then submits /auto-mode-setup. When autoMode entries already exist in settings, a wizard opens asking whether to add to them or start fresh. That wizard is also terminal-only. The reporter says the session stays blocked until Escape is sent to the terminal, which also discards queued user input. They report two long-lived sessions frozen within 24 hours on a dedicated Mac, and relate it to #80806, another terminal-only approval that Remote Control clients do not get. Reported repro conditions: ~/.claude.json has numStartups of at least 5 and autoModeEnvSetup.denials equal to 5, there is no autoMode.environment in settings, and some other autoMode entry (an allow rule) exists. Run one turn in auto mode, then use tmux send-keys to send /rename test followed by Enter. The rename does not happen and the wizard opens, with nothing shown in the Remote Control client. Possible mitigation (reporter-stated, not verified by KFA): setting skillOverrides to {"auto-mode-setup": "off"} reportedly disables the offer and applies to running sessions. The reporter found it by reading the bundle, and it is not documented. Escape in the terminal reportedly recovers a stuck session at the cost of discarding queued input. Status: no maintainer response, confirmed cause, fix commit or release is present in the supplied material, so fix status is unknown. Limitations: this is a single external report, not reproduced by KFA; the repro depends on specific config state; and comment coverage is partial.
