## Title

Chat Completions replay emits invalid sequence for two messages in one tool-call turn

## Body

Finding: In openai-agents-python 0.23.1 (also reported on main at commit 125efa029b4bfd84238bd2c4fd69c3406f802663), the Chat Completions converter Converter.items_to_messages reportedly produces an invalid message order when a Responses-format history contains a function_call followed by two assistant messages and then the function_call_output. Source: https://github.com/openai/openai-agents-python/issues/5342.

Reported symptom (external report, EXTERNAL_REPORT): the first message is merged into the assistant message that carries tool_calls, but the second becomes a separate assistant message placed before the tool result. The resulting sequence is user, assistant(tool_calls), assistant, tool. The reporter says the SDK's own code comment states that this sequence is rejected by the API, and that a message following a refusal in the same tool turn has the same problem. Source: https://github.com/openai/openai-agents-python/issues/5342.

Environment as reported: SDK 0.23.1, Python 3.12, Windows, Chat Completions adapter with a local mock client and no paid API call. The reporter says the behavior is consistent for this input order and that a control history with just one assistant message between function_call and function_call_output converts correctly. The malformed payload was seen both in a direct converter probe and in the payload that Runner.run passed to a mocked chat.completions.create. Source: https://github.com/openai/openai-agents-python/issues/5342.

Related history: the reporter says #4727 was fixed for a single message after a streamed tool call by #4728, and presents this as the remaining multi-message case. We did not have those items among the supplied sources, so this is only the reporter's statement. Source: https://github.com/openai/openai-agents-python/issues/5342.

Expected behavior per the reporter: the two same-turn text messages should be merged into one assistant message with the declared tool_calls, immediately followed by the tool result, while a later assistant turn after the tool result stays separate. Source: https://github.com/openai/openai-agents-python/issues/5342.

Status and limitations: the issue is open; no maintainer response, confirmed cause, workaround, fix commit or released fix appears in the supplied data, and comment retrieval failed. A live HTTP 400 was not measured by the reporter; rejection is inferred from the documented ordering contract. This is a single external report and was not reproduced by KFA. Diagnostic hint only: if a replayed history has more than one assistant message between a function_call and its output, inspect the converted message list for a stray assistant message ahead of the tool message.
