## Title

LiteLLM non-streaming completion reports 0 tokens when provider usage is missing or null

## Body

Finding: In an external report, LiteLLM SDK non-streaming acompletion (mistral/mistral-small-latest, versions 1.96.2 and 1.104.2) returns usage with prompt_tokens=0 and completion_tokens=0 when the provider response omits usage or sets it to null. This looks identical to a provider-reported real zero (https://github.com/BerriAI/litellm/issues/45586). Reported conditions: SDK only, no proxy, no custom callbacks, drop_params or cost-map overrides, deployed via Docker. The reporter replaced only the HTTP transport with a mock (client=AsyncHTTPHandler with httpx.MockTransport), so LiteLLM's own response parsing ran unchanged. They state that three inputs (usage omitted, usage null, explicit zeros) all print (0, 0), and that no field on the returned object, including hidden params, retains the original value (issue body).

Suspected cause (hypothesis, not maintainer-confirmed): the reporter points to main.py attaching an empty litellm.Usage() to the response before parsing the provider reply, so a missing usage keeps the zero-filled default (line 5118 in 1.96.2, line 5443 in 1.104.2). A commenter reads the code the same way (https://github.com/BerriAI/litellm/issues/45586#issuecomment-6080975373). Neither is a maintainer.

Impact per the report: cost tracking based on usage or completion_cost would record a call with missing usage as costing nothing, and a gateway cannot fail calls with missing usage because missing is not visible.

Reporter's workaround: plausibility checks that treat a non-empty prompt with 0 input tokens, or a non-empty answer with 0 output tokens, as a failure. The reporter notes this cannot catch an empty answer whose output count is missing.

Fix status: a commenter said a fix is up as PR #45707 (https://github.com/BerriAI/litellm/issues/45586#issuecomment-6091859702). The same commenter earlier planned to keep the zero default and record the absence of provider usage in hidden params (comment 6080975373). The PR was not supplied or reviewed, and no merge or release is evidenced. Do not treat it as a shipped fix.

Limitations: this is an external report, not reproduced by KFA. The cause is a hypothesis, there was no maintainer response in the supplied thread, and the comment list may be incomplete. Streaming behavior is mentioned only as related and is not covered here. Affected versions beyond 1.96.2 and 1.104.2 are unknown.

"body": "Finding: Open PR #45707 would make a missing provider usage block detectable on non-streaming completion without changing counts (https://github.com/BerriAI/litellm/pull/45707). Per the patch, convert_to_model_response_object adds usage_missing=True to the response's hidden params when the provider body omits usage or sends null (https://github.com/BerriAI/litellm/pull/45707.patch). The zero-filled Usage() default stays, so prompt_tokens and completion_tokens still read 0. An explicit all-zero usage block is left unmarked. The patch adds unit tests for missing, null and real-zero usage, plus two completion tests with a mocked HTTP route. The author says the missing and null tests fail on main; this is author-reported and was not run here. This matches the commenter's announced approach (https://github.com/BerriAI/litellm/issues/45586#issuecomment-6091859702). Status at retrieval: the PR is open, a single commit, with an automated review raising style nits and no approving review shown. The supplied release pages (v1.104.2, v1.106.0-dev.1 to dev.3) were truncated and are only partial coverage, so no released fix is evidenced. Guidance: on 1.96.2 or 1.104.2, the returned usage cannot separate missing from zero, so keep the reporter's plausibility checks (which miss an empty answer with a missing output count). If you try the PR branch, read the flag from the response hidden params and check it with the issue's repro. Because counts remain 0, cost calculation from usage still sees zero unless your code checks the flag first. Limits: non-streaming path only; the flag could change or the PR could be reworked (for example to None counts) before merge.",
