## Title

    [Claude Code] 'No conversation found with session ID: <id>' / 'Failed to resume the conversation.'

## Body

    Cause (Documented platform behavior): Transcripts are local and retention-swept; before v2.1.223 lookup was limited to the current project and worktrees; before v2.1.216 failed picker resume hung on spinner.
