This is an external report; KFA did not reproduce it. Reported behavior: The issue author says the notification branch sends 202 first, then calls writer.send(session_message). On a closed writer this raises ClosedResourceError. The broad POST error handler then tries to send an HTTP 500 after the response has started and tries another write through the closed writer (https://github.com/modelcontextprotocol/python-sdk/issues/3631). The author's reproduction closes transport._read_stream_writer after connect() in JSON response mode and posts the notification via an ASGI transport that raises app exceptions; this simulates a terminated session. Additional external failure reports: One commenter says the same failure occurs on main at 2118f14 and attributes it to the ordering of 202 versus writer.send (https://github.com/modelcontextprotocol/python-sdk/issues/3631#issuecomment-5971523807). Another reports reproduction on the same commit, with the client-visible failure being the already-started-response assertion (https://github.com/modelcontextprotocol/python-sdk/issues/3631#issuecomment-5976607199).
