## Title

LiteLLM streamed usage.cost from OpenAI-compatible aggregators is recorded as USD spend, inflating budgets

## Body

Finding: Per external reports on BerriAI/litellm issue #35829, streamed chat through an openai/-prefixed OpenAI-compatible aggregator can record a non-USD usage.cost value as USD spend. The reports cover LiteLLM v1.98.1 through v1.104.2, and the original issue cites main-latest. Source: https://github.com/BerriAI/litellm/issues/35829. This is an external report only; KFA did not reproduce it. Symptoms: The original reporter says an aggregator returned a cost in nano-dollars and LiteLLM stored it as dollars. That bypassed the configured input and output per-token prices and tripped a $25 max_budget with the message Budget has been exceeded. The reporter says recovery needed a database spend reset and a container restart (https://github.com/BerriAI/litellm/issues/35829).

A later comment on v1.102.0 describes a different aggregator reporting micro-unit costs. A streamed request with a true cost of about $0.0005 logged spend 503, and the non-streamed twin logged correctly. The commenter also reports that the configured pricing was skipped, with breakdown total 0 on the streamed row (https://github.com/BerriAI/litellm/issues/35829#issuecomment-5990919416). A third comment, from 2026-10-09, reports the same on v1.104.2 with an aggregator that uses credits, where 1 EUR equals 1,000,000 credits (https://github.com/BerriAI/litellm/issues/35829#issuecomment-6084940458).

Reported cause (reporter code tracing, not maintainer-confirmed): the streaming helper _stream_builder_response_cost returns a numeric usage.cost unless the provider is on an exception list. That list reportedly contains only xAI. Commenters also say the non-streaming path strips provider-stated cost when custom pricing is configured, while the streaming path does not (https://github.com/BerriAI/litellm/issues/35829#issuecomment-5990919416).

Status: The issue is closed. A commenter says PR #39441 fixed only xAI and gated an OpenRouter shortcut, and says the generic case is still live in v1.104.0. A later commenter says the same on v1.104.2. No released fix for the generic case is evidenced in the supplied sources, and closure alone is not proof of a fix.

Diagnosis: If an aggregator's streamed usage includes a cost field, compare the stored spend with the token-based cost. A gap of many orders of magnitude, with breakdown total 0 on the streamed row only, matches the report. The commenters' proposed fixes are hypotheses: make operator pricing always win, trust reported cost only via an adapter contract, add a sanity bound, or add an opt-out flag.

Limitations: Comment coverage may omit middle comments. The reporters, not KFA, supplied the provider units and the code paths.

"body": "Two commenters report that relabeling an affected aggregator deployment as custom_llm_provider: xai, with api_base unchanged, restored operator pricing on streamed requests (https://github.com/BerriAI/litellm/issues/35829#issuecomment-5990919416; https://github.com/BerriAI/litellm/issues/35829#issuecomment-6084940458). Side effect, per the first reporter: the provider is misreported in analytics. The second reporter says it should not be a long-term solution. Both reports are about v1.102 to v1.104.x. This is an external workaround that KFA has not verified and that may break if the exception list changes. Compare the spend logs for a streamed and a non-streamed request after the change.",
