This is an unconfirmed external report; KFA did not reproduce it. Reported symptom: The original report (2026-10-07, app 26.930.61225) says the @oai/sky import works but the first list_apps call fails after approval. Restarting the app and using a fresh Cloud chat did not help. The same visible Notepad test worked in a local chat (https://github.com/openai/codex/issues/51867). The reporter also says earlier approval-policy rejections were resolved by choosing On request, and that this launch failure is a separate problem (https://github.com/openai/codex/issues/51867).\n\nReported observations on 2026-10-10: Resetting Node REPL did not help, and the local MCP node_repl tool showed the same error. The codex.exe named by CODEX_CLI_PATH in the node_repl config exists and returns its version. The reporter's read-only process inspection of the failing Cloud route shows ChatGPT.exe -> codex.exe exec-server -> node_repl.exe -> node.exe -> codex-computer-use.exe, with CODEX_CLI_PATH and SKY_CUA_NATIVE_PIPE absent from the helper environment.
