    Cause (Documented platform behavior): Before v2.1.282 Claude Code kept the refused block; now it drops earlier thinking and retries once.
    
    Fix status: released_fix (fixed in v2.1.282)
    
    Unknowns:
    - What makes the block data invalid (docs don't say; possibly gateway/provider switch)
