The cause is unconfirmed and no fix is identified. Source: https://github.com/openai/codex/issues/45894 Reported conditions: The original reporter describes a main thread receiving a subagent progress update and then replying to an earlier user prompt that had already been handled. A follow-up comment gives the environment as ChatGPT Desktop (Codex) 26.908.70816 on macOS, released Sep 15, 2026 (https://github.com/openai/codex/issues/45894#issuecomment-5693977496). Windows reports: One commenter reports that after an unrelated inter-agent request and a context compaction, the parent issued a second create_thread call for an old, completed objective (https://github.com/openai/codex/issues/45894#issuecomment-5710882930). A later report says current work and a cross-thread reply succeeded, yet the local final answer led with an older topic. A contextCompaction event occurred earlier in that turn, and the installed Windows package was 26.928.3736.0 (https://github.com/openai/codex/issues/45894#issuecomment-6038910647). The newest Windows report says the inbound message was visible and acknowledged, but the final answer addressed an older task. It rules out transport loss and notes that app version and model were not captured (https://github.com/openai/codex/issues/45894#issuecomment-6100275839). A related variant: a commenter reports a stale prior-turn final answer after an ordinary user message, with no visible current-turn write call (https://github.com/openai/codex/issues/45894#issuecomment-6091995532). That commenter says compaction or networking is not claimed as the sole cause. Diagnostic implication: Reports consistently suggest that successful delivery, successful tool calls and a nonempty final answer can coexist with a final answer bound to the wrong request. A suggested check, from the reporter's view and not a verified fix, is to compare the final answer with the latest inbound event. Compaction precedes the mismatch in two reports, but one reporter says a shared cause is unconfirmed. Limitations: These are external user reports, not KFA reproductions.
