## Title

    Proposed fix: [pytest] 'import file mismatch: imported module ... has this __file__ attribute ... which is not the same as the test file we want to collect' — duplicate test basenames without __init__

## Body

    Recommended action: Add __init__.py files to make test dirs packages (names become tests.unit.test_utils), or rename files uniquely, or use --import-mode=importlib (tests then cannot import each other's helper modules). Delete __pycache__ when paths changed.
    
    Option: Make test directories packages or use importlib mode [evidence: official_recommended_action]
    Applies when: Repos with duplicate test file names
    Steps:
    1. Add __init__.py to tests/ and subdirs (or rename files)
    2. Or set addopts = --import-mode=importlib and move shared helpers into the app package
