    Proposed fix: [MCP Kotlin SDK] requests fail with -32603 'Server is busy: too many in-flight messages' (notifications dropped) when a peer floods concurrent requests (default 256 in-flight handlers)

    Recommended action: Throttle client concurrency or raise maxInFlightHandlers (must be >= maxConcurrentHandlers) in protocol options; retry rejected requests with backoff.
