    ## Cause
    The 2026-07-28 basic specification requires protocolVersion and clientCapabilities on every request _meta and mandates -32602 / HTTP 400 when any required field is missing. TS SDK inbound classification emits Invalid _meta envelope for protocol revision 2026-07-28 when validation fails.
    
    ## Steps
    1. Add _meta.io.modelcontextprotocol/protocolVersion with the active revision string.
    2. Add _meta.io.modelcontextprotocol/clientCapabilities object (empty object acceptable if no capabilities).
    3. Verify proxies do not strip _meta or rename keys.
    4. Align MCP-Protocol-Version header with the body envelope when using Streamable HTTP.
