    Proposed fix: [VS Code JS/TS] 'The JS/TS language service crashed 5 times in the last 5 Minutes.' / 'immediately crashed 5 times. The service will not be restarted.' — tsserver OOM at 3072 MB default

## Body

    Recommended action: Disable listed plugin extensions first; for large repos raise js/ts.tsserver.maxMemory (old: typescript.tsserver.maxTsServerMemory) and, above 4 GB, set js/ts.tsserver.node.path to a system Node; enable js/ts.tsserver.log to capture the crash; narrow the project via tsconfig include/exclude.
    
    Option: Rule out plugins, then raise tsserver memory [evidence: official_recommended_action]
    Applies when: Repeated tsserver crashes in large workspaces
    Steps:
    1. Disable extensions named in the crash message and retry
    2. Set "js/ts.tsserver.maxMemory": 8192 and "js/ts.tsserver.node.path": "node" (needed above 4 GB)
    3. Run 'TypeScript: Restart TS Server'
    4. If still crashing, set js/ts.tsserver.log to verbose and inspect the log
    Expected: tsserver stays up.
