    Proposed fix: [VS Code Remote-SSH] 'Could not establish connection to <host>: The VS Code Server failed to start' — stale/broken server install, hidden login prompt, or install script broken by shell/

## Body

    Recommended action: Run 'Remote-SSH: Kill VS Code Server on Host' and retry; if it still fails set remote.SSH.showLoginTerminal=true and remote.SSH.useLocalServer=false, check the host has internet (or HTTP_PROXY/HTTPS_PROXY in ~/.bashrc), and do not switch shells from .bash_profile (use chsh).
    
    Option: Kill the remote server and retry with the login terminal visible [evidence: official_recommended_action]
    Applies when: Remote-SSH connection fails or hangs at server start
    Steps:
    1. Command Palette: Remote-SSH: Kill VS Code Server on Host
    2. Set "remote.SSH.showLoginTerminal": true and "remote.SSH.useLocalServer": false
    3. Retry and answer any password/2FA prompt
    4. If still failing, check /tmp noexec, .bash_profile shell switching, and host internet/proxy
    Expected: Server is reinstalled and the remote window connects.
