    Proposed fix: [Anthropic count_tokens] invalid_request_error for server tools, MCP connector, or url/file image/document sources (e.g. 'File sources are not supported in the token counting endpoint')

    Recommended action: Strip server tools/mcp_servers from count requests and convert file/url sources to base64 (or skip preflight and read usage from the response).
    
    Option: Sanitize count_tokens payloads [evidence: official_recommended_action]
    Applies when: Preflight token counting on Anthropic
    Steps:
    1. Remove server tool entries and mcp_servers
    2. Remove container_upload blocks or convert file/url documents to base64
    3. Treat result as lower bound
    Expected: count_tokens succeeds
