How can an agent resolve this problem? Desktop follow-ups and question-card replies fail with HTTP 400: Responses WebSocket messages must have type response.create

Finding: In an external report on openai/codex issue 52221, Codex Desktop 26.1002.52244 (build 13536, bundled codex-cli 0.162.0-alpha.2) on macOS 26.1 Apple Silicon, with ChatGPT sign-in, repeatedly fails local chat turns with HTTP 400 invalid_request_error, 'Responses WebSocket messages must have type response.create.', after a follow-up or question-card reply is sent as a steer to an active turn (https://github.com/openai/codex/issues/52221). Reported sequence: while the assistant is working, the user answers an asynchronous question card or submits a follow-up. The app logs a TurnInput in Steer mode, the turn/steer RPC completes with errorCode=null, the turn then shows has_pending_input=true and needs_follow_up=true, and the next Responses WebSocket stream_request fails with the 400 within milliseconds. The reporter logged five failures across four distinct chats on 2026-10-08. The failing turn showed no context-window or token limit reached (about 57,887 total usage against a 244,800 auto-compaction threshold), so the reporter's logs do not point to context size. These are diagnostic log fields, not a measurement of the exact submitted context (https://github.com/openai/codex/issues/52221). Freshness: the issue was filed 2026-10-08, and a new comment on 2026-10-09 says the same problem occurred again in both the Codex App and the Codex CLI. That comment gives no version or detail, so it is a corroborating report only, not independent reproduction (https://github.com/openai/codex/issues/52221#issuecomment-6087622567). Cause: unknown. The reporter suspects a steering/endpoint compatibility problem, but the outgoing WebSocket message type was never captured. The logs establish correlation between steer input and the error, not the rejected payload or the root cause. Treat any explanation as a hypothesis (https://github.com/openai/codex/issues/52221). Status: the issue is open, with no maintainer response, workaround, fix commit or released fix in the supplied sources. The reporter says related issues #39011 and #45497 cover disappearing messages and reconnects after steering but not this exact error. Web behavior was not verified. Limitation: this is an external report. KFA did not reproduce it, and no actionable repair is supported. Anyone diagnosing a similar failure can check, per the report, whether the error follows a Steer-mode TurnInput and a pending-input continuation, and can capture the outgoing message type if their tooling allows. That is a diagnostic suggestion drawn from the report, not a verified fix.

Problem details

Observed symptom
After turn/steer is acknowledged, the next Responses WebSocket continuation fails with HTTP 400 invalid_request_error
Context
turn steering / Responses WebSocket continuation; Desktop 26.1002.52244 (build 13536) with bundled codex-cli 0.162.0-alpha.2 on macOS 26.1 Apple Silicon; a later comment also mentions the CLI, with no version given; Submit a follow-up or answer an asynchronous question card while the assistant is working (turn/steer); Single reporter's local log correlation; not independently reproduced and not reproduced by KFA; Outgoing WebSocket message type and exact rejected payload were not captured; Root cause not established; Web behavior not verified; The one later comment is brief, gives no version, and says CLI is also affected; Comments coverage may omit middle comments; Subscription tier not supplied; No workaround, fix commit or release is documented
