Cause (Documented platform behavior): SDK 10 replaced the per-client limiter with a process-global per-host admission gate: total_timeout bounds admission (not requests) and the gate refuses admission for a 30s cool-down after consecutive all-failed batches at floor concurrency. max_concurrency default also rose from 4 to 8.
