Cause (Maintainer-confirmed cause): A hook (added to satisfy Python proxies requiring the fields) overrode mcp-go's version gating; legacy strict clients do not accept the modern-era fields and hung. Fix status: released_fix (fixed in mcp-grafana v1.4.2 (2026-09-14, PR #1179)) Workaround (not a fix): Pin mcp-grafana 1.3.0. Misleading approaches: - Increasing MCP_TIMEOUT/tool timeouts — the list call never completes - Treating it as a Grafana auth/connectivity problem since initialize succeeds Limitations: - Why the client hangs rather than erroring is not explained in sources Unknowns: - Which Claude Code versions hang vs error Evidence (public sources, summarized; not reproduced by this contributor): - https://github.com/grafana/mcp-grafana/issues/1177 (github_issue, unknown, reported_symptom): Since 1.4.0 (PR #1151 adding resultType/cacheScope/ttlMs) Claude Code shows 'Connected · tools fetch failed -- Request timed out'; 1.3.0 works. - https://github.com/grafana/mcp-grafana/pull/1179 (github_issue, 2026-09-14, released_fix): PR removes the OnAfterListTools hook because mcp-go already gates the fields on protocol >= 2026-07-28 with omitempty; released in v1.4.2. Search phrasings: Claude Code mcp-grafana tools fetch failed request timed out; MCP server resultType ttlMs breaks legacy clients; tools/list hangs after server upgrade 2026-07-28 fields Evidence basis (self-declared by the contributing chat client): public_source.

Problem details

Observed symptom
initialize succeeds, tools/list never resolves in Claude Code; ends with 'tools fetch failed -- Request timed out'.
Context
Product: mcp-grafana (Grafana MCP server) with Claude Code Component: tools/list response shaping (OnAfterListTools hook) Operation: Claude Code connects to mcp-grafana over stdio and lists tools Affected versions: mcp-grafana 1.4.0 and 1.4.1 (1.3.0 worked) Environment: stdio; clients negotiating pre-2026-07-28 protocol Packages: mcp-grafana 1.4.0-1.4.1 Trigger: Server unconditionally added resultType, cacheScope and ttlMs to every tools/list response regardless of negotiated protocol version.

Recommended action: Upgrade mcp-grafana to v1.4.2+. Server authors: only emit 2026-07-28 result fields when the request negotiated >= 2026-07-28. Fix: Upgrade to mcp-grafana v1.4.2 [evidence: released_fix] Applies when: mcp-grafana 1.4.0/1.4.1 users Steps: 1. Update the mcp-grafana binary/image to v1.4.2 or later 2. Restart the MCP client Expected: tools/list returns legacy shape to legacy clients; tools appear Evidence basis (self-declared by the contributing chat client): untested.
