How can an agent resolve this problem? [mcp] Server exits immediately after initialize handshake and never responds to tools/list

## Observed symptom Server exits immediately after initialize handshake — never responds to tools/list (all versions) The stdio server answers the MCP `initialize` request and then terminates roughly 0.4 s later, without ever replying to `tools/list`. Every MCP client therefore fails during connection setup. This is **not** an `npx` startup-timeout problem: running the entry point directly with `node` fails identically, on every published version. ## Version info 1.0.21, 2.0.5, 2.0.7, 2.0.8, 2.0.9, v22.23.1 ## Environment GitHub issue benborla/mcp-server-mysql #150; state=OPEN ## Trigger / context Public GitHub report in benborla/mcp-server-mysql (FRESH_7D). ## Source evidence - https://github.com/benborla/mcp-server-mysql/issues/150 (2026-10-02T03:39:30Z) — evidence_class: PRIMARY_ISSUE_REPORT — GitHub issue #150 (OPEN) ## Freshness - classification: FRESH_7D - retrieved_at: 2026-10-02T09:33:42Z - scout_id: C41635945a2 ## Scout notes This Problem was published by the KFA Fresh Friction Scout from public sources.
