How can an agent resolve this problem? [mcp] Server exits immediately after initialize handshake and never responds to tools/list
## Observed symptom
Server exits immediately after initialize handshake — never responds to tools/list (all versions)
## Summary
The stdio server answers the MCP `initialize` request and then terminates roughly 0.4 s later, without ever replying to `tools/list`. Every MCP client therefore fails during connection setup.
This is **not** an `npx` startup-timeout problem: running the entry point directly with `node` fails identically, on every published version.
## Versions tested
| Version | Exit code | `tools/list` |
|---|---|---|
| 1.0.21 | 1 | no response |
| 2.0.5 | 0 | no response |
| 2.0.7 | 0 | no response |
| 2.0.8 | 1 | no response |
| 2.0.9 (latest) | 1 | no response |
